July 23, 2013 - School Athletic Physicals  

The physicals will take place at the Shaw Medical Building, 1111 Kentucky. OMC will offer free athletic physicals to student athletes. All students will be required to submit a signed consent form and current insurance information from a parent or legal guardian. Consent forms may be picked up at West Plains High School downstairs office, at No. 3 Parkway Shopping Center or at OMC Rehabilitation Services, located on the second floor of the Shaw Medical Building, 1111 Kentucky, West Plains. 

Students should arrive according to the firstletter of their last name: 

4:30 to 5 p.m. A to E

5 to 5:30 p.m. F to I

5:30 to 6 p.m. J to M

6 to 6:30 p.m. N to Q

6:30 to 7 p.m. R to U

7 to 7:30 p.m. V to Z.
